Graphic Design Request Form


To request a new design, please fill out all required fields and attach the approved content before submitting the online form at least 2-4 weeks in advance. Please keep in mind that any additional edits will delay the design process.

Materials for an event will be designed after the event is approved by the Campus Activity Board (CAB). All event materials must include date, time, and location of the event and contact.

Testimonial materials should include name (for all banners), title (for staff and faculty), program of study (for students and alumni), graduation year (for alumni). Media release form should be signed for all testimonials before Wed Design Form Submission.

    Content Timeline determine upon number of proposed pages for final design. Confirmation Email Notification to Requester once content approved by the Communications Manager.

  • Step 3: Material Information

    Please select the type of material you require. Only one project may be selected per request. Please submit separate forms for every project.
  • Step 4: Printing

  • Please note that the submission of this form does not guarantee delivery of materials at the time requested. However, the Media Department seeks to support all FXUA initiatives in the timeliest manner possible. The time needed to complete a design will be altered during certain times of the year. Please give us as much advance notice on a request as you can. Thank you!
